Research Reveals New Way to Administer Anesthetic in the Mouth

A study published in Colloids and Surfaces B: Biointerfacesreveals how the dentist could provide anesthetic using a tiny electric current instead of a needle.

The researchers behind the study, from the University of São Paulo, say their new findings could help improve dental procedures and bring relief to millions of people who are scared of needles. It would also save money and avoid contamination and infection, they say.

"Needle-free administration could save costs, improve patient compliance, facilitate application and decrease the risks of intoxication and contamination," explained Professor Renata Fonseca Vianna Lopez, one of the authors of the study from the University of Sao Paulo in Brazil. "This may facilitate access to more effective and safe dental treatments for thousands of people around the world."

Dentists often have to carry out invasive and painful procedures in the mouth. To minimize patients' discomfort, dentists use anesthetics that block the pain, which are administered using needles. However, many patients are extremely afraid of these injections, resulting in them postponing and even canceling visits to the dentist.

For these patients, an additional step is needed: dentists first give them a topical painkiller to reduce the pain—and associated fear—caused by the needle. This can come in the form of a hydrogel, ointment or sprays; the most common are hydrogels that can contain lidocaine and prilocaine.

In the new study, the researchers investigated a way of getting these topical anesthetics into the body more effectively, to see if they could replace needles altogether. They found that applying a tiny electric current—a process called iontophoresis—made the anesthetics more effective.

The researchers first prepared the anesthetic hydrogels with a polymer to help it stick to the lining of the mouth. They added two anesthetic drugs, prilocaine hydrochloride (PCL) and lidocaine hydrochloride (LCL). They tested the gel on the mouth lining of a pig, applying a tiny electric current to see if it made the anesthetic more effective.

The anesthesia was fast-acting and long-lasting. The electric current made the prilocaine hydrochloride enter the body more effectively; the permeation of the anesthetic through the mouth lining increased 12-fold.

The researchers say the technology has applications not only in dentistry anesthesia, but also in other areas such as cancer treatment.

"Over the last few years, our research group has been working on the development of novel drug delivery systems for the treatment of several skin and eye diseases," said Prof. Lopez. "The skin and eyes pose challenges for drug delivery, so we have focused on improving drug delivery in these organs using nanotechnology, iontophoresis and sonophoresis, which is permeation using sound waves."

The researchers now plan to develop an iontophoretic device to use specifically in the mouth and do some preclinical trials with the system.

The two were honored for their work on the Dental Wellness Plan, the dental component of the Medicaid expansion that was launched in May 2014.

The Gies Awards are the dental industry's pre-eminent recognition for individuals and organizations that exemplify the highest standards in oral health and dental education, research and leadership.

Hans Geiselhöringer appointed President of Nobel Biocare

Zurich, Switzerland – As of January 1, Hans Geiselhöringer has taken over as President of Nobel Biocare. 

Since 2011, Hans Geiselhöringer has served as Executive Vice President of Global Research, Products and Development, shaping a highly competitive product and innovation pipeline. Prior to that he was Executive Vice President, Global Marketing and Products from 2010–2011 and Head of NobelProcera and Guided Surgery from 2009–2010. Hans joined Nobel Biocare as Head of NobelProcera in 2008 and was appointed a member of the executive committee in 2009.

TEAM and Lab Tech Program at AO San Diego

Lars Hansson will talk about the TEAM and Laboratory Tech Program at The Academy of Osseointegration’s (AO) 2016 Annual Meeting in San Diego. The dental team, as well as laboratory technicians, can gain insight into the implant process. Scheduled on Saturday, February 20, the programs demonstrate the team approach to implant dentistry.The TEAM program (formerly the Allied Staff Program) is designed to provide comprehensive information for the entire staff, from the dentist, to the implant treatment coordinator, to the hygienists, assistants, and receptionists.

Lars Hansson, CDT, FICOI, is excited that the team concept is an underlying theme for the Laboratory Technician Program. Designed to help clinicians, laboratory technicians, specialists, and the rest of the dental team better communicate and understand each other’s part in the process, Hansson thinks the program will help “bridge the gap” between the team in implant dentistry. “I want them to see how much easier the process will be and how much more of the treatment they will be part of because today a lot of technicians are afraid of speaking up,” Hansson explained. “Being more involved in patient treatment, protocol, and the treatment plan inspires a lot of technicians to become better.”

“These meetings are always exciting because you meet peers and people that you’ve looked up to, and that you’re going to learn from. And you listen to people that have far more knowledge than you do. You will get a step closer by going to the meeting,” Hansson said.

#AOSanDiego Offers Extra CE Credit

Each new Hands-On Workshop Session for the Academy of Osseointegration’s (AO) 2016 Annual Meeting offers the opportunity to earn three continuing education (CE) credit hours. Plus, hands-on learning is among the most effective methods for retaining information.

“Approximately 85% of the nerves in the hands go directly to the brain, thus making a “hands-on” approach is the most effective reinforcement of a visual exposure you can experience,” Dr. Stephen Parel, a presenter for one of the sessions, explains.

The Hands-On Workshop Sessions are February 17th, 2016, the day before the Annual Meeting officially begins. They will be at the Marriott Marquis San Diego Marina Hotel. Each of the programs runs from 1 to 4 p.m. Certificates of attendance will be sent to the participants following the meeting.

Dr. Parel believes the CE courses offered before this year’s meeting will not only benefit clinicians, but also patients.

This is especially true in terms of confidence and clarity for procedures that the clinician may not have attempted before in an actual clinical setting,” he said.

UAMS Seeks Approval for Feasibility Study of Dental School

The University of Arkansas for Medical Sciences wants to study the feasibility of adding a dental school, the state's first, and on Friday a legislative council will decide whether to authorize $50,000 for the study.

On Tuesday, the Legislative Council's review subcommittee backed a plan by UAMS to use $50,000 in private donations for the first phase of a feasibility study by the Academy for Academic Leadership of Atlanta.

UAMS spokesperson Leslie Taylor said if the university receives final legislative approval for the study on Friday, it will proceed with the feasibility study, which could take six months.

After UAMS receives results from the study, it will make the determination on whether to proceed, she said.

A dental school at UAMS would be the state's first. UAMS operates a satellite medical school in northwest Arkansas, and the state has two law schools at the University of Arkansas, Fayetteville, and the University of Arkansas at Little Rock. But it's never had a dental school or a veterinary school.

UAMS does operate a dental residency program at its dental clinic. Many prospective dentists from Arkansas attend the University of Tennessee Health Science Center in Memphis for their dental training.
