Development of Drugs for Local Treatment of Oral Conditions

Seoul, Republic of Korea – At the 94th General Session & Exhibition of the International Association for Dental Research, researcher W. Peter Holbrook, University of Iceland, presented a study titled “Development of Drugs for Local Treatment of Oral Conditions.” The IADR General Session was held in conjunction with the 3rd Meeting of the IADR Asia Pacific Region and the 35th Annual Meeting of the IADR Korean Division.

Several medications commonly prescribed for oral mucosal administration are actually intended for transdermal application. Many conditions affecting the oral mucosa require frequent or long-term treatments and some treatments are systemically applied. Clinical resistance and patient intolerance of such treatments may develop. There is thus a constant need to address these problems through the development of less side-effect prone drugs.

Researchers at the University of Iceland have developed several new formulations for topical treatments of oral mucosal conditions and carried out appropriate clinical trials. The main area of this research has been the inhibition of matrix metallo-proteinase activity using topical doxycycline, initially in order to treat aphthous ulceration. The antimicrobial compound monocaprin was also tested for activity against herpes virus and, in combination with doxycycline, was developed in an active formulation for treating cold sores (canker sores). Monocaprin also has anticandidal activity that has been evaluated among geriatric patients with denture stomatitis.

Topical application of doxycycline was very effective in promoting healing of mucosal lesions. Monocaprin reduced counts of Candida rapidly and significantly. The results of these clinical studies have been very promising when compared to the conventional treatments available in Iceland and abroad. Stability of the active components has recently been effectively addressed and current research is aimed at different types of drug delivery systems in order to optimize drug delivery to the local mucosal site. This includes drug release time and mucoadhesive capacity of the formulation, thereby attempting to develop more disease-specific drug delivery systems.

This research was funded by grants from the Icelandic Research Council (Rannis) and the University of Iceland.

Researchers Study HPV Infection in Leukoplakia in the Last Decade

Seoul, Republic of Korea – At the 94th General Session & Exhibition of the International Association for Dental Research, researcher Jonas Sundberg, University of Gothenburg, Sweden, presented a study titled “Increased Expression of p16 in Oral Leukoplakia the Last Decade.”

During the last two decades an increase in incidence of oropharyngeal carcinomas related to human papilloma virus (HPV) infection has been reported. Although a casual association between lekoplakia, oral cancer, and HPV has been suggested in several reports, no conclusive evidence has been presented. To assess the presence of HPV, the tumor suppressing protein p16 is commonly used as a surrogate marker for HPV infection. The aim of the present study is to investigate whether p16 expression in tissue specimens from lekoplakia has increased during the last decade, which may indicate an increase in incidence of oral HPV infection.

A patient cohort with tissue specimens obtained between 1992 and 2001 and a cohort where specimens were obtained between 2011 and 2015 and with histopathological diagnoses of benign hyperkeratosis or hyperkeratosis with dysplasia were examined. Immunohistochemistry was performed using a monoclonal antibody (clone E6H4; Roche Ltd) visualizing p16. Digitalized images of sections were obtained and a semiquantitative scale (0-5) was used to enumarate p16 expression in oral epithelium.

In the 2011-2015 cohort lekoplakia epithelium showed a significant increase (P = .02) in grade 4 and 5 p16 expression in comparison with lekoplakia epithelium in the 1992-2001 cohort. In lekoplakia presenting with dysplasia, grade 4 and 5 p16 expression also increased significantly (P = .0001) over time when comparing the cohorts. The results indicate an increase in HPV infection in lekoplakia during the last decade. Researchers are further investigating the presence of HPV DNA utilizing polymerase chain reaction is presently undertaken.

This research was funded by the Regional Research Council, Region Västra Götaland, Assar Gabrielsson Foundation and the Swedish Dental Society.

 

America’s ToothFairy Announces 2016 Youth of the Year Award Winners

Charlotte, NC – June 24, 2016) – America’s ToothFairy: National Children’s Oral Health Foundation® is proud to announce its 2016 Youth of the Year Award winners. This award recognizes high school students who have made exceptional efforts to raise awareness of the importance of children’s oral health and wellness in their community.

This year’s winners, Sabrina Herbert, Sara Rodriguez, Iris Valadez, and Caroline Vasquez, are members of the HOSA: Future Health Professionals chapter at Southwest High School in El Centro, CA, and actively participate in the #MySmileMatters Youth Movement. Launched in 2003, #MySmileMatters is an initiative of America’s ToothFairy engaging nearly 600,000 youth in oral health education and advocacy.

Under the leadership of Jackie Valadez, RDH, this year’s award winners demonstrated exceptional understanding of the issues pertaining to children’s dental health and wellness, while also holding an array of creative events to improve oral health literacy in their school and community. Specific activities included oral health educational presentations, social media advocacy, planting of Smile Gardens, and water demonstrations.

The awards, which include a $250 scholarship, will be presented at the HOSA 39th Annual International Leadership Conference in Nashville, TN, today. The students will also present highlights from their #MySmileMatters activities at a conference seminar for fellow HOSA students.

All #MySmileMatters participants utilize the #MySmileMatters National Youth Engagement Plan, which guides youth and their adult allies in integrating dental health education and learning into their schools and communities. The 2016-2017 National Youth Engagement Plan has the theme “Cavities are NOT Supposed to Happen” and focuses on “changing beliefs, changing behavior, and changing the environment” of children’s dental health and wellness. OSAP Announces New Boards of Directors

June 24, 2016 (ATLANTA, GA) — The Organization for Safety, Asepsis and Prevention (OSAP), a community of clinicians, educators, researchers and industry representatives who advocate for safe and infection-free delivery of oral healthcare, has announced the members of the 2016-2017 OSAP Board of Directors and the 2016-2017 OSAP Foundation Board of Directors.

The new OSAP Association Board is chaired by Mike Smurr, Director of Marketing, Merchandise, for Patterson Dental. Vice Chair is Kathy Eklund, Director of Infection Control and Occupational Health, and Research Subject and Patient Safety Advocate at Forsyth Institute in Boston. Dr. John O'Keefe, a general dentist who is Director of Knowledge Networks and Editor-in-Chief with the Canadian Dental Association, is the Immediate Past Chair. Marguerite Walsh, Vice President, Product Merchandising for Henry Schein Dental, will serve as Treasurer, and Karen Gregory, Director of Compliance and Education for Total Medical Compliance, will serve as Secretary.

Addressing Antibiotic Resistance in Periodontal Treatment

June 22, 2016- For years the Centers for Disease Control and Prevention has been warning about the rise in antibiotic resistant bacteria and the challenges those bacteria pose to our health. In a recent publication on the website Perio-Implant Advisory, Dr. Duane Keller explains how bacteria become resistant to antibiotics and points a way to a non-antibiotic therapy for gum disease that follows the guidelines for chronic wound care and relies on oxidative and oxygenating agents.

Bacterial communities (biofilms) that cause infections leading to gum disease lie deep in periodontal pockets, the spaces between the teeth and gums which are hard to reach with toothbrush, rinse, or floss. The pocket is a unique space for chemotherapeutic modalities of care because it can be topically accessed, but keeping medicaments deep in the pocket for a significant amount of time was the most significant challenge for dentists until the invention of the Perio Tray® (Perio Protect, St. Louis, MO). 

While it looks a lot like a mouth guard, the Perio Tray® is specially fabricated with internal peripheral seals and extensions to place and hold medication deep in periodontal pockets.Research shows that the Perio Tray® can deliver medication into pockets as deep as 9mm (1-3mm is healthy). 

Hydrogen peroxide, even at a low concentration of 1.7%, has proven to be a highly effective antimicrobial when delivered and held deep in the pockets with the Perio Tray®.
